## A Session That Looked Ordinary and Wasn't

My client Susan (a pseudonym) was on a video call with me, adjusting the angle of her iPad so I could see her leg on the [Variable Incline Plane (VIP)](https://betterknees.com/blog/the-best-investment-for-knee-or-hip-pain).

We were at level two, using 21% of her bodyweight, the same setting we'd used during her previous session.

She did five squats with both legs.

And five with her right leg alone.

Then we got to the exercise I was actually watching for: [eccentric squats](https://youtu.be/D0RgLBmI0uA) with her left leg.

Because her left knee was even more painful than her right.

Thirteen days earlier, that same exercise had stopped her at five pain-free reps. The sixth one had hurt.

This time, she did five, rested, then added three more.

Later in the same session, we returned to it and she completed six additional reps.

Fourteen pain-free reps on the same leg, at the same small percentage of her body weight.

Nearly three times as many less than two weeks later.

Susan didn't experience that as dramatic.

She described it as "pretty modest, but it's so gratifying that there hasn't been any fallout."

That gap, between what the data showed and what Susan felt, is the whole point of this article.

## Why Knee Pain (and Knee Progress) Don't Match What You Feel

Most people assume that if their knee is getting stronger, they'll feel it clearly and immediately.

That assumption is understandable.

It's also usually wrong.

Muscle gives you real-time feedback.

You can feel fatigue building set by set.

But the tissue inside your knee, your cartilage and synovial lining, doesn't work that way.

Unlike muscle, [cartilage has no nerve supply](https://pmc.ncbi.nlm.nih.gov/articles/PMC3445147/), so it can improve, or get overloaded, without an immediate signal.

This is [the same reason knee pain often shows up a day or two after activity instead of during it](https://betterknees.com/blog/why-knee-pain-shows-up-after-exercise-even-when-it-feels-fine-at-the-time), rather than in the moment.

That delay cuts both ways.

It's why symptoms can lag behind a bad load.

It's also why real improvement can happen well before a client feels confident it's real.

Susan's own words captured this perfectly: "I won't really feel confident until I see those squats getting easy enough to add weight levels. Then I'll feel like maybe this can work."

But her knee had already answered that question.

## The Inside Out Knee Health Framework, In One Session

Susan’s session is a clean example of all three phases working together.

Phase 1: Improve the internal joint environment .

Before any loading happened, Susan did Quad Sets, Sitting Sliders, and ‘Tailgaters.’

Phase 3: Return to functional, real-life load.

Susan had recently done unavoidable yard work that put more force through her knees than her routine did.

She recovered in two days instead of a week.

## What This Looked Like in Practice (Not Generic Exercises)

If two full rounds feel fine, she adds one rep the next time.

Two rounds of 10 reps becomes two rounds of 11, then 12, and so on, until her next Better Knees Coaching Session.

That last detail matters.

Progressing load isn't about pushing harder.

It's about precision: progressive reps for a knee that's ready for it, and holding steady for a knee that isn't.

## Common Mistakes People Make With Knee Pain Like This

### Mistake 1: Waiting to feel confident before trusting the plan

Susan is disciplined and detail-oriented, and she still wanted to feel transformed before she'd believe her knee was improving. Most people do. But confidence is a lagging indicator. If you wait for it, you'll often abandon a plan that was already working.

### Mistake 2: Strengthening muscles that are already strong

Susan's previous physical therapist recommended more quad-strengthening. Her response was blunt: her legs were already strong, "like tree trunks." Her muscles were never the limiting factor. This is an extremely common mismatch. Muscle strength lets you complete a squat with good form. It has almost nothing to do with whether your knee can tolerate that squat afterward without irritation. Building real Squat Load Tolerance, rather than more muscle, is what closes that gap.

### Mistake 3: Treating unavoidable activity as either forbidden or fine

Susan felt like she should have entirely avoided yard work that needed to happen. That's the wrong frame. The better question is how quickly her knee recovers afterward. Two days of mild irritation followed by full resolution is a very different signal than a week of setback. ## The Reframe: Progress Isn't the Absence of Doubt

The common assumption is that people can feel their own healing in real time, and that doubt means something isn't working.

The more accurate picture is that the tissue inside your knee changes on its own timeline, one that often runs ahead of your confidence, not behind it.

One told her that knee replacement was inevitable, and that she "couldn't really make it any worse."

[That framing, that osteoarthritis is a one-way deterioration best managed by surgery, is common](https://www.cdc.gov/arthritis/osteoarthritis/index.html), but it isn't the full picture.

The same tissue that "can't get better" is the tissue that was just able to withstand nine more pain-free reps in under two weeks.

### How can a knee get stronger before it feels different?

Cartilage and synovial tissue have far less nerve supply than muscle, so meaningful adaptation can happen without producing a noticeable in-the-moment sensation of change. Tracking a specific measurement, like pain-free reps at a known load, reveals progress that subjective "how does it feel" check-ins often miss.

### What is Squat Load Tolerance, and why track it instead of pain level?

Squat Load Tolerance is the percentage of bodyweight a knee can handle in a squat during five repetitions without pain. Pain is a lagging, inconsistent signal, especially since [knee symptoms often appear a day or more after the load that caused them](https://betterknees.com/blog/why-knee-pain-shows-up-after-exercise-even-when-it-feels-fine-at-the-time). A direct measurement removes the guesswork.

### Why didn't strengthening her quads fix Susan's knee pain?

Because her quads were never very weak. Muscle strength determines whether you can complete a movement with good form. It does not determine whether the internal structures of the knee can tolerate that movement afterward. Those are two separate systems, and generic PT protocols frequently treat them as one.

### Is it normal for pain-free reps to increase this quickly?

Individual responses vary, and this is not a guarantee. But when load is measured and progressed in small, deliberate increments rather than guessed at, meaningful within-week changes in tolerance are a plan working as intended, not an outlier.

### Why does unavoidable activity like yard work sometimes cause a temporary setback?

Any activity that exceeds current Squat Load Tolerance, even briefly, can cause temporary irritation. What matters more than avoiding that activity entirely is how quickly symptoms resolve afterward. A two-day recovery reflects a very different (and improving) tolerance than a week-long setback would.

### Do I need a Variable Incline Plane (VIP) to measure my knees?

A [VIP](https://betterknees.com/blog/the-best-investment-for-knee-or-hip-pain) is the tool that allows for precise, repeatable measurement of Squat Load Tolerance at home, which is often central to the Inside Out Knee Health approach. It's a different tool than a leg press, which cannot accurately isolate or measure this the same way.

If you don’t have access to a VIP, you can still get a rough estimate of your Squat Load Tolerance using a standard chair. If you can sit down onto a standard chair and stand back up five times with good form, loading each leg with approximately half of your body weight, and you experience no knee discomfort during the movement or afterwards, your Squat Load Tolerance for each knee is likely at least 50% of your bodyweight. But if either knee experiences pain or discomfort at any point during the movement or afterwards — even if the discomfort is mild or tolerable — your squat load tolerance is likely less than 50% of your body weight.

### Is it too late to avoid a knee replacement?

Every case is different, and this isn't medical advice for any specific situation. But many clients who were told surgery was their only remaining option have gone on to [build measurable knee joint strength and capacity to enjoy their desired activities](https://pmc.ncbi.nlm.nih.gov/articles/PMC3140275/) – sometimes years after being told their knees "couldn't get any worse" as a reason to proceed with surgery rather than a reason to try internal knee strengthening first.
